Richard Sloggett This is one of the scenarios for which we created the Community Extensions support. If you enable that feature in the Build Profile (perhaps in a custom "For Review" Build Profile), the CHMs that you generate will include the ability for people viewing the CHM to add comments and notes to individual topics (both in the conceptual and reference topics). In Document! X, you can view the comments that users have created whilst you are authoring, and also get access to a complete list of comments covering the whole project from a link on the Community Features page in the Build Profile editor.
You can view a sample of some content that includes the Community features here: http://www.innovasys.com/products/hs2012/samples/webhelp/webframe.html
There is also a movie providing an introduction to the Community Features here: http://www.innovasys.com/movies/viewmoviefeed.aspx?id=4489f046-52b0-4e65-add0-3af878209ee0
