Missing TOC and Index headings when generating a booklet

I am generating a PDF booklet along with the HTML help. The TOC adn index are included well. However, the two headings (Table of Content and Index) aer missing. The index entries just start after the last page of my help file without a heading at the top of the page.

Also, is it possible to have a TOC entry for the Index heading?

I am using automatic index entries. Does that make any difference?

Could you please send a copy of your project to support@innovasys.com so we can investigate - it's not an issue that we are aware of at present.
